Accident, Injury & Near Miss Reporting

General Information

All incidents/accidents must be reported to a supervisor as soon as possible following the incident/accident. The main reporting forms used are:

  • UBC Faculty & Staff Incident/Accident Report
  • WorkSafeBC Form 6A- Worker’s Report of Injury or Occupational Disease to the Employer
  • WorkSafeBC Form 7A- First Aid Report (completed only when first aid is administered by a qualified FA attendant)
  • UBC Student & Visitor Incident/Accident Report

As of January 1 2008 workers who are injured at work and miss time from work as a result can now report their injuries to WorkSafe BBC calling the Teleclaim Contact Centre by dialing 1-888-WORKERS (1-888-967-5377), Monday to Friday, 8 am to 4 pm.  Through Teleclaim, callers speak to a WorkSafeBC representative, who completes an injury report on the workers behalf, explains the claim process, and helps obtain the services to assist with recovery and return to work.
